I vote for 'really badly confused'. The only parents I've ever known that call each other "Mom" and "Dad" were my grandparents, who did it for their own amusement. It confused people when she addressed my grandfather as Father Joe, which was not his first name.

"Mommy" isn't terribly common, but my parents call each other "Mom" and "Dad" sometimes. I think it's acceptable.
